FORM INSTRUCTIONS: Fill out each form field with the information requested. If you paid with someone else's card then enter their information instead of yours, we have to be able to match the payment to the registration.

When asked for your first name, your correct and entire first name is needed, not a partial name, not initials, not a business name. This is the same for your last name. The email address will only be used if our staff need to contact you. Then, select a username and password.

The final portion and the most important part is your acknowledgement that you read the terms, that you understood the terms and that you agree to comply with the terms, if all three of those statements are true then check the box and submit the form, if you do not agree with the three statements then click the POLICIES link at the top of this page and read the terms until you do agree.
